The Conference registration fee includes attendance at educational sessions, breaks, CME credit certificate, Thursday evening Reception and Behind the Scenes with DEAPA. To register, please complete and return the Conference Registration Form in this brochure or register on-line at www.deapa.com/conference. Early Bird registration is available through 12/31/11!
Faculty Disclosure Policy
All faculty members participating in this educational program are expected to disclose to the program audience any significant financial interest and/or or other relationship with the manufacturers and providers of commercial products and/or services discussed in their presentation and their relationship or financial interest in any commercial contributor to the educational activity.
Cancellation/Refunds
If you must cancel, please notify by phone or fax. Conference registration fee, minus a $50 administrative fee, will be refunded if notice of cancellation is received prior to January 6, 2012. No refunds will be granted after January 6th.

Sunday River is offering conference attendees discounted lift ticket rates: $53 weekday (Mon-Thur; 9am-4pm); $58 weekend 1/2 day (12N-8pm); and $64 Weekend Full Day (8am-8pm).
